Your First Visit

Let’s get to know you and your smile.

Your first appointment gives us time to review your dental and medical history, discuss your concerns, and learn about your goals. We’ll explain our findings and talk through any recommended next steps.

What to Bring:

  • Bring a photo ID

  • Your dental insurance card if applicable

  • A current list of medications

  • Completed patient forms (see download below)

If you have recent dental records or X-rays, contact us before your visit so we can help arrange a transfer.

Patient Forms

Save time at your first visit by completing your new patient paperwork in advance. Please include your current contact information, medical history, and medications.

Prefer to complete your forms in person? Our team can provide them when you arrive. Please arrive a few minutes early.

Dental Insurance

Help understanding your benefits.

Dental coverage varies by plan. Contact our office before your appointment so we can review your insurance information and explain how your benefits may apply.

Benefit estimates aren’t a guarantee of payment.
